Event Request Form

Pastors/Directors fill out this form for their events.

This form is for every event and meets all needs, including facilities, video/audio, childcare, staff support, cleaning, hospitality, communications, etc. This form is not for small private meetings such as Flourish Groups, Life Groups, or small ministry meetings or trainings. 

After this request form is filled out, all communication regarding an upcoming event will happen in the Teams channel for that event. As a general guideline, you should place a request 6 weeks before your event date.

(Men? Women? Students? High Schoolers? etc.)

The Hospitality Team does All-Church events only, such as Outreach, Memorials, Leadership, and Life Group events. Each department needs a team of volunteers for departmental events. Connie will help people from many departments with how to and location of items but can not fully support the events as there are just too many.
